Requirements

  • Must be 18 years of age.

  • Completed Application Form.

  • Provide Two Satisfactory References

  • Provide the following documents of Criminal Background Forms​

    • FBI Fingerprint Background Check

    • PA Criminal Background Check

    • PA Child Abuse Clearance (if applicable)

  • Two Step Screening (TB Test) or Chest X-Ray and Physical Examination

  • Provide Proof of Residence of 2 Years or more (one of the options)

  • PA Driver's License ID or PA Photo ID

  • Social Security Card (signed)

  • Power of Attorney Form (if applied)
